Our membership form is an online form which you can fill in on a phone, tablet or computer. If you would like a paper form, or need BSL support to fill in the form please email our Parent Liaison Officer, Cheryl McBride (parentsupport@lollipopyork.org.uk).
This is a new version of the form that we are testing, so if there are any problems please do let us know! It is made using Cognito Forms which provides an extra layer of encryption to protect your information. It will be processed and stored according to our Privacy Policy which can be found at the bottom of the page.
